As an Acute Medical Registrar working across AMU, ED and Same Day Emergency Care, I routinely manage the unselected acute take, typically reviewing 20–30 patients per shift with consultant support, often practising independently in high-pressure situations. My day begins with a structured handover, where I identify high-risk patients, pending investigations and escalation decisions, and I prioritise unstable patients first — particularly those requiring senior review, procedures such as lumbar puncture or pleural aspiration, or complex decision-making around ceilings of care. On AMU I lead a small team of junior doctors, allocating cases by acuity, supervising assessments and ensuring management plans are safe and timely, with close communication with nursing coordinators, bed managers and specialty teams to facilitate flow and early discharge planning. Throughout the shift I reassess deteriorating patients, escalate appropriately to ICU when needed, and ensure clear documentation and handover to maintain patient safety and continuity of care.
