What struck me was that the examination itself was the investigation — that in neurology the history and the hands still determine the answer more than any scan does. That is what draws me intellectually, but I have deliberately tested whether I want the rest of the job too. I spent two days in the MS clinic and a week in a general neurology outpatient clinic during a taster eighteen months ago, precisely because I wanted to know how I would find managing conditions I could not cure. I audited door-to-needle times for thrombolysis across 74 patients, found the delay sat almost entirely in the interval between CT and decision, and after we moved to a direct-to-CT pathway the median fell from 71 to 49 minutes; that was presented at the regional stroke meeting in March. I have also taught the foundation doctors' neurological examination session for four terms, with structured feedback averaging 4.7 across 41 respondents, and I have been an ABN trainee member for two years. What I found hardest was functional neurological disorder — explaining a diagnosis that patients often experience as disbelief — and I sought out clinics to get better at it rather than avoiding them.
