On the acute medical unit I noticed VTE assessments were completed in only 62% of admissions within 24 hours against the NICE standard of 95%. I formed a small team with the nursing lead and pharmacy. We mapped the process, surveyed staff, and identified two drivers: the assessment was buried in the clerking proforma and there was no nurse prompt at handover. Our PDSA cycle one introduced a stamped checklist on the front sheet; cycle two added a board-round prompt; cycle three embedded it in the electronic clerking template. Compliance rose to 94% over twelve weeks and has sustained. I presented at the directorate governance meeting and the work has been adopted on three further wards. The biggest learning was the value of frontline engagement before designing any intervention.
