My priority is patient safety, my colleague's wellbeing, and a fair process. I would first verify the facts rather than relying on hearsay and consider whether handover gaps had already caused harm — if so, I would complete a Datix and inform the consultant immediately. I would then speak to the colleague privately, somewhere quiet, using open questions: 'I've noticed handovers are being missed, can you help me understand what's happening?' This often uncovers workload, health or personal issues, and they may need support rather than discipline. I would clarify the expectation, agree concrete actions and offer to help — perhaps adjusting rota patterns or signposting to occupational health or the PSU. I would document the conversation in a reflective entry. If the behaviour continued or there was a safety concern, I would escalate to the educational supervisor and clinical lead, following the local underperforming trainee policy.
